Meet Rosy
For any value-based care initiative to be successful, the patient must be top of mind. As we learn from each other during the Summit, our attention will continue to turn to Rosy - our patient avatar developed exclusively for this program.
Rosy is 65 years old when we first meet her. Rosy recently retired, and loves to spend her days traveling, gardening, and spending time with her family and friends. During the Summit, we will follow her along her care journey as she faces a difficult diagnosis with a long path to recovery.
By the end of the Summit, we will wish Rosy well, knowing that patients like her will receive better care as leaders in the industry gain a greater understanding for how to implement effective value-based initiatives.
Value Based Care Timeline for Rosy
Rosy goes to her annual checkup and finds a cancerous lump in her breast.
Rosy is connected with a care team that helps her to manage her medications, schedule appointments, connect with community resources, and eat nutritious foods.
Rosy receives a cancer diagnosis and undergoes several months of testing and treatment.
Rosy is cancer-free, but faces a perilous journey ahead as she begins to recover.
Rosy has been cancer-free and is thankful for her care team that provided excellent care and supported her in her long recovery.
